Criminal Background Checks are required to be completed on ALL staff, counselors and volunteers every year. It is the responsibility of the program sponsor to ensure that the forms are submitted to HSC Risk Management for review and approval prior to the start date of each program. There is a cost of $6.50 per check, which will be deducted from your designated program account. The form can be found on the Forms and Resources Page.
