Learning Collaborative

Many projects in RHP 8 test inventive models of care that move away from historical systems of treatment, and toward a more integrated and coordinated approach. These inventive models focus on the patient’s whole-person needs and health outcomes. As RHP 8’s Performing Providers implement advances, it is important they learn from others successes and challenges, as well as identify best practices that can be widely imitated.

The DSRIP Transformative piece of the 1115 Medicaid Waiver encourages bringing together the various Performing Providers in the RHP to learn from each other and improve their clinical practices through continuous quality improvement (CQI).

RHP 8’s Learning Collaborative focuses on:

  1. Sharing knowledge and expertise;
  2. Improving patient and care-giver experience; and
  3. Implementing evidence-based guidelines.

Learning Opportunities

RHP 8 Learning Collaborative Event

Did you know April 1, 2014, was the mid-point mark for the 1115 Waiver in the state of Texas? In the past 2 ½  years, Performing Providers in RHP 8 have been busy implementing new, transformational projects in Central Texas. The Anchor team is excited to bring together stakeholders in the region for our second, face-to-face learning collaborative event this summer!

As projects are now being implemented throughout the state it is important DSRIP and UC Providers, IGT Entities, and other community stakeholders come together to share best practices, lessons learned, and success stories in order to increase successes as we all head into DY4 and DY5.

The Anchor team is happy to welcome our keynote speaker, Ramdas Menon, Ph.D., Deputy Director of Medicaid Health IT at the Texas Health & Human Services Commission; his presentation will focus on “Big Data, Big Health: The Link between Data Collection Management and Combating Obesity”.

Additionally, we are happy to welcome four, RHP 8 DSRIP project leads to highlight their projects in our communities which are focused on either decreasing obesity, increasing data collection, or both! Please review the agenda and RSVP if interested in attending by sending Gina Lawson asn email at: glawson@tamhsc.edu.

Regional Work Group

RHP 8 Behavioral Health & Primary Care Cohort

The RHP 8 Behavioral Health & Primary Care Cohort aims to bring together DSRIP Providers and other RHP stakeholders who have similar projects, goals, and interests in identified focus areas.

The objective of the Cohort is to foster collaboration through focused quality improvement activities in an effort to promote efficiencies and improve healthcare delivery and access throughout the region.

The first Cohort development and organization face-to-face meeting is planned for Thursday, July 31, 2014, at the Texas A&M Health Science Center campus in Round Rock.

Join us for our second learning collaborative presentation Tuesday, September 23, 2014, from noon-1 p.m. The Anchor team will update the Regional Timeline page with more information as the date gets closer.
