Learning Collaborative

Collaborative learning occurs when a group of individuals or organizations share information and experiences in an effort to drive change and seek improvement for a common purpose. The RHP 17 Learning Collaborative seeks to achieve regional impact through shared learning, community engagement, and celebration of success. We believe there can be great benefit to the region at large when stakeholders come together in activities designed to foster collaboration and dialogue geared toward learning and continuous quality improvement.

The design of the RHP 17 learning collaborative is a version of the CMS-recommended IHI model, modified to accommodate our smaller, more rural region. The collaborative structure is very similar to that of our sister region, RHP 8 in Round Rock, and our larger regional counterpart in Houston, RHP 3, to allow participation across regions by interested providers.

"Individual" and "Regional" mechanisms for learning and sharing, fed by "Core" components, work in a continuous cycle to promote quality improvement and positive regional impact as illustrated below.

Learning Opportunities

Regional Opportunity: HHSC Statewide Learning Collaborative Summit

HHSC hosted the statewide summit on September 9-10, 2014 in Austin. Keynote presentations included information on Medicaid quality strategies and the waiver's impact on mental health policy and strategy in Texas, as well as panels on collaboration, integrated care models and data metrics. Breakout sessions covered a range of topics including QPI, Category 3 baselines, midpoint assessment, waiver evaluation, CQI, rural opportunities and risk adjustment. Peer-to-Peer Opportunity: Joint Lunch & Learn presentation with RHP 8 (Round Rock Region) on Appreciative Inquiry

Region 8 will be giving a lunch-hour presentation on Appreciative Inquiry from 12-1 p.m. on Tuesday, September 23, 2014. Visit the RHP 8 Learning Collaborative page for more information and contact either Anchor Team ([email protected] or [email protected]) to register.

Regional Work Groups

Cohort work groups aim to bring together DSRIP providers and other RHP stakeholders, who have similar projects, goals and interests in identified focus areas. The objective of these groups is to foster collaboration through focused quality improvement activities in an effort to promote efficiencies and improve health care delivery and access throughout the region.

RHP 17 is in the process of developing two work groups with focus on the following areas:

  • Behavioral Health & Primary Care
  • Care Transitions and Navigation

Initial work group development and organization is underway. Anchor is working with volunteer co-facilitators to finalize cohort group packets for planning/development with kick-off meetings to follow in late September/early October.
