Learning Collaborative

Collaborative learning occurs when a group of individuals or organizations share information and experiences in an effort to drive change and seek improvement for a common purpose. The RHP 17 Learning Collaborative seeks to achieve regional impact through shared learning, community engagement, and celebration of success. We believe there can be great benefit to the region at large when stakeholders come together in activities designed to foster collaboration and dialogue geared toward learning and continuous quality improvement.

The design of the RHP 17 learning collaborative is a version of the CMS-recommended IHI model, modified to accommodate our smaller, more rural region. The collaborative structure is very similar to that of our sister region, RHP 8 in Round Rock, and our larger regional counterpart in Houston, RHP 3, to allow participation across regions by interested providers.

"Individual" and "Regional" mechanisms for learning and sharing, fed by "Core" components, work in a continuous cycle to promote quality improvement and positive regional impact as illustrated below.

Learning Opportunities

Peer-to-Peer Opportunity: RHP 8 (Round Rock Region)

Region 8 will be hosting a Learning Collaborative event on August 20, 2014, at the Georgetown Chamber of Commerce featuring a presentation by HHSC on big data. Visit the RHP 8 Learning Collaborative page for more information.

Regional Opportunity: RHP 17 Biannual Learning Collaborative Event

RHP 17 will host the second face-to-face event of DY3 on August 12, 2014, at the Annenberg Presidential Conference Center on the Texas A&M campus. Stakeholders will have the opportunity to hear presentations by Melissa Rowan of the Texas Council of Community Centers, Kevin Deramus of Washington County EMS, and Andrew Karrer of Montgomery County Public Health Department. Additionally, posters for some of the region's DSRIP projects will be displayed and attendees will participate in a tabletop exercise related to quality improvement methodologies. Regional Work Groups

Cohort work groups aim to bring together DSRIP providers and other RHP stakeholders, who have similar projects, goals and interests in identified focus areas. The objective of these groups is to foster collaboration through focused quality improvement activities in an effort to promote efficiencies and improve health care delivery and access throughout the region.

RHP 17 is in the process of developing two work groups with focus on the following areas:

  • Behavioral Health & Primary Care
  • Care Transitions and Navigation

Initial work group development and organization is underway with a goal of holding initial group meetings in mid-August 2014 once the DY4-5 change request and Category 3 outcome measure submission processes are complete.
